As mandated by the Florida High School Athletic Association (FHSAA), every Shorecrest athlete must be cleared to participate for practices and for all team competitions.

  • All forms mentioned below must be submitted via the FHSAA Athletic Clearance Website. We do not accept paper athletic forms in order to expedite and streamline the clearance process.
  • All athletes must be cleared through the Athletic Clearance System at least 72 HOURS BEFORE tryouts or their first practice.
  • Use the Athletic Clearance Guide to prevent mistakes and to ensure all information is included when completing an Athletic Clearance.

Athletic Clearance Guide

The clearance process requires the following:

  • A completed FHSAA physical (EL2) form signed by the student’s doctor (sample below)

  • Signed Insurance/Liability/Heat/Concussion/Cardiac Arrest (EL3) forms. (These forms are available to digitally sign in the Athletic Clearance system.)

In addition, Shorecrest requires the following forms to be completed by a parent and a student:

Once the courses are completed, students must upload the completion certificate from each course to their athletic clearance account. The completion certificate must have the STUDENT’S name on it, not their parent’s.
